FELLOW OF THE ASSOCIATION
Award History

This award recognizes individuals who have provided outstanding professional service and/or scientific achievement.  A nominee must have been an individual who has practiced in the profession for a minimum of 5 years. The recipient shall be a current member in good standing. Fellowship may be given for a significant single contribution or for a series of contributions.  Supporting documentation may consist of any of the following areas:

            1. Innovative teaching techniques or innovative curriculum development
           2. Innovative clinical techniques or clinical programs
           3. Innovative supervisor techniques or supervisory programming
           4. Outstanding contributor to literature or research
           5. Service to OSLHA including service on committees, the Executive Board, Legislative
               Council, or in other significant areas
           6. Outstanding administrative abilities
