ELWOOD CHANEY OUTSTANDING CLINICIAN and STATE CLINICAL ACHIEVEMENT AWARD
Award History

This award recognizes an individual's significant accomplishments in the advancement of clinical practice and service in speech-language pathology and/or audiology.  One of the nominees who will be receiving this award will be chosen to become the state of Ohio's nominee for the Louis M. DiCarlo State Clinical Achievement Award presented by the American Speech-Language-Hearing Foundation (ASHF) at the ASHA convention.  Nominees must be OSLHA Active or Life members in good standing.  Nominees are not limited to any particular type of clinical achievement or activity.  The following information is considered when nominating an individual for this award and support documents must include information from at least three areas.

1)   Membership and involvement in a local organization or state organization, 

2)   Achievement with a single client or group of clients,

3)   A specific clinical service or training program,

4)  A specific achievement in a state or local organization or government agency activity,

5)  A specific accomplishment in clinical teaching, research or administration,

6)  Innovative contribution to work setting.
