Call for Nominations Nomination Deadline: January 15, 2009 The process for obtaining nominations for Honors of the Ohio Speech-Language-Hearing Association has begun. Please consider nominating those individuals that you feel best represent the ideals of our profession in clinical practice, research, teaching and service to the community. HONORS OF THE
ASSOCIATION This award is a one-time/life-time award. Recipients of this award are recognized for distinguished and significant lifetime contributions and leadership to the field of speech, language, and hearing through clinical practice, teaching, research, administration and/or legislative activity. Individuals must have practiced in the profession for a minimum of 10 years. It is the highest Honor the Association can give. The recipient shall be a current member in good standing who has already been awarded OSLHA Fellow. An exception to the above would be: an in-state or out-of-state professional who is not an OSLHA member, but who the Association feels is worthy of Honors because the person has provided exceptional service to the professions of speech-language pathology and the clients we serve. Nominations must include the following information about the nominees and Support Documents must include information from at least 3 of the following areas: 1. Research and/or publications contributing basic or applied knowledge of the profession 2. Teaching in Communication Sciences & Disorders 3. Clinical services to children/adults with communication disorders 4. Service to OSLHA and/or affiliates 5. Specific achievement within the profession FELLOW OF THE
ASSOCIATION This award recognizes individuals who have provided outstanding professional service and/or scientific achievement. A nominee must have been an individual who has practiced in the profession for a minimum of 5 years. The recipient shall be a current member in good standing. Fellowship may be given for a significant single contribution or for a series of contributions. Supporting documentation may consist of any of the following areas:
1. Innovative teaching techniques or innovative
curriculum development ELWOOD CHANEY
OUTSTANDING CLINICIAN AND STATE CLINICAL ACHIEVEMENT AWARD This award recognizes an individual’s significant accomplishments in the advancement of clinical practice and service in speech-language pathology and/or audiology. One of the nominee’s who will be receiving this award, will be chosen to become the state of Ohio’s nominee for the Louis M. DiCarlo State Clinical Achievement Award presented by the American Speech-Language-Hearing Foundation (ASHF) at the ASHA convention. Nominees must be OSLHA Active or Life members in good standing. Nominee’s are not limited to any particular type of clinical achievement or activity. The following information is considered when nominating an individual for this award and support documents must include information from at least three areas: 1. Membership and involvement in a local or state organization 2. Achievement with a single client or group of clients 3. A specific clinical service or training program 4. A specific achievement in a state or local organization or government agency activity 5. A specific accomplishment in clinical teaching, research or administration

Innovative contributions to work setting AWARD OF APPRECIATION This award is presented to individuals, agencies, businesses, or organizations in recognition of significant contributions that have enhanced OSLHA's ability to serve its membership or to enrich the lives of the communicatively impaired. This award may recognize a member or non-member for dedication in either a broad or focused area of service. The following information is considered when selecting the recipient(s) of this award: 1. Contributions to OSLHA's ability to serve its membership or to enrich the lives of the communicatively impaired must be considered significant; 2. Contribution can be demonstrated as a single, significant event or may be considered significant because of a series of events or longstanding contributions STUDENT RECOGNITION AWARD This award recognizes an undergraduate or graduate student in the field of Speech-Language Pathology or Audiology who has exhibited the following: 1. Outstanding leadership
qualities 3. Membership and involvement in NSSLHA 4. Membership and involvement in OSLHA.
This award recognizes an Ohio legislator for contributions which have or will affect our profession in the area of communication disorders. This legislator's work will have far-reaching effects in a positive manner to not only the speech and hearing impaired but the general public.
