Consumer Award Nomination

A COMMUNICATION SUCCESS

Do you know a consumer of speech, language or audiology services who is successfully communicating with others, achieving well with their disability? Do you know of someone who works tirelessly, promoting the rights of those with communication disabilities? If the answer to either or both of these questions is yes, read on. The Ohio Speech-Language-Hearing Association would like to help you recognize the individual or group.

The Consumer Liaison Committee is sponsoring a project to recognize these successes. For complete information, see the article about OSLHA Consumer Awards below. Mail recommendations for these honors, no later than January 22, 2008.

CONSUMER LIAISON PROJECT


PROJECT TITLE:   OSLHA CONSUMER AWARDS

GOAL:    To recognize, with awards from the Ohio Speech-Language-Hearing Association, individuals or groups who:

A.  as consumers of speech, language or audiology services are coping with and successfully achieving with their disabilities (focus being on ability),

or

B.  as consumer advocates are promoting the rights of individuals with communication disabilities. These may be parents or spouses of communicatively handicapped individuals or organized consumer groups.

ELIGIBILITY:    Any OSLHA MEMBER or AFFILIATE GROUP may nominate a distinguished consumer or consumer advocate (individual or group) to receive the award. The person making the nomination, individually or for an affiliate, must hold current membership in OSLHA.

CRITERIA:     Winning nominations for the awards will be based upon:

A.  Outstanding achievement of a client of a speech pathologist or audiologist in one of these age categories: children or adult (up to 2 awards). The person making the nomination should submit no more than a one page summary that would include: 

    a) a brief background of the nominee's communication handicap, 
    b) the reason for the nomination and, 
    c) an objective summary of the nominee's name and age group should be included, as well as the name, address and phone number of the person making the nomination. Material, such as copies of newspaper or magazine articles, awards, etc., can be included to support the nomination.

OR

B.  Outstanding work of parents or spouses of communicatively handicapped individuals or of organized consumer groups in promoting the right of individuals with communication disabilities (Up to 2 awards). The person making the nomination should submit no more than a one page summary that will include:                                                                     
   a) the reason for the nomination and, 
   
b) an objective summary of the nominee's accomplishments.
     c)  The nominee's name should be included, as well as the name, address and phone number of the person making the nomination. Materials, such as copies of newspaper or magazine articles, awards, etc., can be included to support the nomination.

DEADLINE:     Mailed by January 22, 2008

AWARDS TO WINNERS:    To be given at the OSLHA Convention in Columbus, March 7, 2008.
