SPEECH-LANGUAGE HEARING ASSOCIATION The
Ohio Speech-Language-Hearing Association is approved by the Continuing
Education Board (CEB) of the American Speech-Language-Hearing Association (ASHA)
to provide continuing education activities in speech-language pathology and
audiology. This issue of Hearsay
(Volume 15, No. 2) is offered for .2 CEUs (Intermediate level, Professional
area). Continuing Education Provider approval does not imply
endorsement of course content, specific products, or clinical procedures. How to Earn Continuing Education Units (CEUs) This CE Activity is offered to OSLHA Members ONLY. v Register
for CEUs for this issue of Hearsay
(Vol. 15, No. 2) by completing the
continuing education participant form.
This
issue was mailed to all OSLHA 03/04 members.
OSLHA will not send out additional
copies of the journal/articles. “Hearsay: Focus on Autism Spectrum Disorders” v
Read
each of the five articles that have been selected as part of this continuing
education experience. These
articles are stipulated in the next pages where learner objectives and
multiple choice questions for each article may be found. v
Complete
the test answer sheet and ASHA CEU Participation form. If you
are unable to download the ASHA Participation Form, please send the
following information with your test and evaluation: v
Complete
an evaluation form to assess your learning experience. v
Include
a self-addressed stamped envelope or your current e-mail address so that you
can be notified of your test results. v
ASHA
CEUs will be submitted bi-annually, 6/30/04 and 12/31/04.
v
In
a LARGE envelope (9x12) please submit to the address below:
a completed ASHA CEU Participation Form, completed answer sheet,
and self-addressed stamped envelope or email address by June 30, 2004
and/or December 31, 2004. All forms received after the deadline will not be processed. v
Individuals
receiving a score of 80% or above on the assessment and those completing the
ASHA CEU Participation form will be credited with .2 CEUs. v
All
