ASHA CONTINUING EDUCATION CREDIT (CEUs)
The Ohio Speech-Language-Hearing
Association is approved by the Continuing Education Board (CEB) of the
American Speech-Language-Hearing Association (ASHA) to provide continuing
education activities in speech-language pathology and audiology. This
issue of Hearsay (Volume 17, 2005) is offered for .2 CEUs (Intermediate
level, Professional area). ASHA CE Provider approval does not imply endorsement
of course content, specific products, or clinical procedures.

How to Earn Continuing Education Units On-Line (CEUs)
CEUs
for the OSLHA HEARSAY journal are available to OSLHA members ONLY.
Members have received a copy of the journal by mail.
HEARSAY, Vol. 17, 2005 can also be found in the "Members Exclusively"
section of this website.
Read each of the following five articles that have been selected
for
this continuing education experience.

Complete the on-line assessment form. Print your completed assessment
form once you have obtained a score of 80% or higher. You may take the
assessment as many times as needed.

In a LARGE envelope (9x12) please submit to the address below the
following items: a completed ASHA CEU Participation Form (if you're on
the ASHA CE Registry), completed assessment form and evaluation form by
December 31, 2005. All forms received after the deadline will NOT
be processed.
Individuals receiving a score of 80% or above on the assessment
will receive .2 CEUs.
