Ohio Model Procedures Bring New Forms (2005) The Model Procedures for the Education of Children with Disabilities 2005-06 brought new forms for school special educators and SLPs this fall. These questions have been asked by SLPs about the new forms: 1. Must parents attend MFE/IEP meetings at their child’s school? When conducting MFE and IEP team meetings as well as other meetings, the parents of a child with a disability and the district may agree to use alternative means of meeting participation, i.e. video conferences, telephone/conference call. There is a place to check which of these will be used on the Parent Information Form (PR-02). 2. Are there any changes on the IEP form (PR-07)? Yes, there are changes in the language used on the new IEP form in these sections:
· “give consent for change of placement” or · “give consent for special education and related services specified in this IEP except for…” or · “do not give consent for a change of placement as identified in this IEP” or · “revoke consent for Special Education Service.” o Parents check that they received a Notice of Procedural Safeguards and a Copy of IEP o Parents can check that they have “attended/participated at the IEP team meeting but do not agree with the special education and related service specified in this IEP”. o There is no longer any re-evaluation information on this page, i.e. last MFE and next IEP shall occur by…”.
