AFFILIATE SUPPORT GRANT

GOAL:     To provide financial support as an incentive to OSLHA Active Affiliates, Probationary Affiliates, and/or proposed Affiliates to stimulate professional activity throughout the State.

ELIGIBILITY:     Project support applications will be accepted from any Active Affiliates, Probationary Affiliates, and/or proposed Affiliates.  The individuals facilitating the project and making application must hold current membership in OSLHA.

CRITERIA:     Proposals will be awarded based on the following criteria:
(Proposed Affiliates must include a proposed copy of by-laws with application)

    1.     Relatively of the project to OSLHA's Long Range Plan  and the promotion of the professions of Speech Language Pathology and Audiology. (50 Points)

    2.     Opportunity for project to stimulate activity that will enhance the quality of speech/language and audiology services to communication disabled individuals in geographic area. (25 Points)

    3.     The opportunity for the project to increase future professional activity in the affiliate or geographic area. (25 Points)

    4.    Timeline for completion of project.

BUDGET:     OSLHA has set aside $4,000.00 for this project. The amount of financial support given will depend on the amount of applicants and the projects awarded.

FOLLOW-UP ACTIVITIES:  All awarded Grant Recipients will:

• Submit the outcome of their project by the OSLHA June Legislative Council meeting of the following calendar year.
  
• Submit a written final report summarizing the project activities, documentation of all expenditures, and outline of the program outcomes, by the June Legislative Council meeting of the following calendar year.
  
• Submit a brief article to the OSLHA Business Office for Communication Matters newsletter by June 10 of the following calendar year.
  

DEADLINE:    
January 28, 2009 for the 2008/2009 fiscal year.
