If you do not have your assigned user ID/password, please submit a written request to the Board office via mail, fax or e-mail.
Updating your contact information will ensure that you receive your renewal notice in a timely manner. Additionally, all licensees are required to notify the board in writing through the U.S. mail, email or fax of any change of name, place of business or mailing address within thirty days of said change.
