Professional & General Liability Insurance
Insurance Coverage is available to
Current OSLHA Members ONLY, who work or live in Ohio or any Adjoining State
(Allied Professionals and Consumers are not eligible)
OSLHA Members electing to participate in the OSLHA Professional & General Liability Insurance coverage receive the following benefits:
- The
payment of up to $1,000,000 each claim on your behalf in the event that
a suit is filed or the insurance company determines negligence is
established.
- $1,000,000 each claim / $3,000,000 fiscal year aggregate, which covers professional services as defined in the policy.
- $1,000,000 each claim / $2,000,000 fiscal year aggregate,which covers general injury and damage as defined in the policy.
- A
trained professional adjuster, employed by the insurance company claims
department, investigates all allegations, responds to complaints and
provides legal defense in the event of suit.
- Company
employed or appointed defense counsel will defend you in the event of a
covered suit, whether or not the allegations have merit.
- All reasonable legal defense costs are paid in addition to policy limits for covered claims.
- Reasonable
expenses incurred by you and up to $100 per day is paid for your actual
loss of earnings because of time off work due to a claim or suit.
- Out of court settlements are paid up to the policy limits.
- No deductible applies.
- All descriptions of coverage are subject to actual policy contract.
The
Professional & General Liability Insurance is an occurrence type policy issued through the
Cincinnati Insurance Company. A master policy is on file with OSLHA.
Members receive confirmation of insurance coverage electronically with 3 weeks of purchasing coverage.
Which Type of Insurance will meet YOUR needs?
EMPLOYEE Liability Insurance
An
"employee" is one who is paid a specified salary or wage from which the
employer deducts Federal Income and Social Security Taxes, and one who
does not engage in any private practice or contract work.
Fiscal Year Premium (July 1 - June 30) $50.00
PART-TIME PRIVATE PRACTICE Liability Insurance
A
person who is in "part-time private practice" is self employed and
receives fees for services from which Federal Income and Social
Security Taxes are not deducted. These services may be provided on a
part-time basis (less than 30 hours per week) only.
Fiscal Year Premium (July 1 - June 30) $50.00
EMPLOYEE & PART-TIME PRIVATE PRACTICE Liability Insurance
This
insurance coverage is for the person in the "employee" setting who is also
engaged in part-time private practice as described in the program
above.
Fiscal Year Premium (July 1 - June 30) $70.00
FULL TIME PRIVATE PRACTICE Liability Insurance
A
person who is in "Full Time private practice" is self employed and
receives fees for services from which Federal Income and Social
Security Taxes are not deducted. These services may be provided on a Full Time basis (30 or more hours per week). Legally established entities are not eligible for this coverage.
Fiscal Year Premium (July 1 - June 30) $100.00
STUDENT Liability Insurance
As
a student, you are responsible for the results of your own actions.
This liability coverage is very important during your educational
activities.
Fiscal Year Premium (July 1 - June 30) $45.00
Application
for Liability Insurance is included on the OSLHA Online Membership
Application Form. You must be a member of OSLHA to purchase liability
insurance and can purchase both on your application form. Please indicate on the form the type of insurance you
need or contact us at anytime to add insurance to your existing membership.
